[kbd] add qwerty cz.map

a cz user converted the qwertz cz.map to the qwerty version.  ive attached the 
file which can be placed into the qwerty subdirectory, as well as the patch 
that was used to convert the qwertz/cz.map to qwerty/cz.map for reference.
